There's a couple of ways to do it, this Whos Online script should be able to get you started in the right direction

basically you keep track of how long it's been since a user last accessed the system, and guesses that after 15 mins they're offline. It's never a "live" system as it could spark thousands of requests to the server at once, which would only take it down
